Person Centred Care in age care

 

Aim: 

 

To empower aged care professionals with the knowledge, skills, and mindset needed to embed person-centred care practices aligned with the 2025 Aged Care Quality Standards, ensuring dignity, autonomy, and cultural safety for every older person.

Course Description: 

 

This course explores the practical implementation of person-centred care in aged care settings, guided by the 2025 Aged Care Quality Standards. Through ten structured lessons, learners will deepen their understanding of human rights, build meaningful relationships with older people, and develop inclusive, respectful care practices. The course blends theory, reflection, and real-world application to support a culture of continuous improvement and compassionate service delivery.

Learner Outcomes:

By the end of this course, participants will be able to:

  • Explain the principles of person-centred care and their connection to the 2025 Standards

  • Apply Standard 1: The Person, to ensure older people’s voices drive care decisions

  • Communicate with empathy, clarity, and cultural sensitivity across diverse populations

  • Create inclusive environments that promote independence, belonging, and wellbeing

  • Engage families, carers, and advocates as partners in care

  • Monitor care quality through feedback, reflective practice, and continuous improvement
